Are you at risk for diabetes?
If you think you are, or have been diagnosed as Pre-Diabetic, please join us for an informative and interactive program to reduce your risk. Learn about risk factors, exercise choices and healthy eating suggestions. Also included will be a pedometer and a blood test that measures your sugar levels over three months at the beginning and end of the series.
Pre-registration is necessary, space is limited. Day or evening classes are offered at Monroe or Dundee.

Diabetes Self-Management Education Program
Learn how to effectively manage diabetes and adopt changes to improve overall health. The program includes an individual assessment with a diabetes educator to review risk factors, self-care skills, discuss personal concerns, and develop your educational plan.
Group instruction follows and includes the following topics: living with diabetes, basic nutrition, treatment options, and managing health-related risks. The American Diabetes Association and Michigan Department of Community Health certify this program. Insurance plans may cover the cost of education. A physician prescription is necessary. Pre-registration is necessary. Morning, afternoon and evening classes offered monthly at the Monroe site. Dundee schedule varies.

Insulin Pump Therapy
If you are considering an insulin pump, ready to get started pumping, or already using a pump and would like to improve control, this program is for you. Options include: Introduction to Pump Therapy, Introduction to Carbohydrate Counting, Living with a Pump, Pump Start, Advanced Pumping and Advanced Carbohydrate Counting. Insurance plans may cover the cost of education. A family member or friend may accompany you at no additional cost.

Diabetes Education Individual Consults
- Monitor training
- Learn how to use your blood glucose monitor, establish a testing schedule and identify target glucose levels.
- Insulin training
- Learn how to properly use insulin, insulin delivery options and safe use of insulin products to meet personal needs.
- Pattern management
- What does those sugar levels mean? Learn how to review glucose readings and discover problem-solving techniques to improve your self-care skills and better manage diabetes.
- Gestational Diabetes
- To understand the impact of diabetes during pregnancy, learn self-care skills and meal planning techniques, take this informative 2- hour session.

Diabetes Exercise Program
This program is offered to persons with diabetes involved in the Diabetes Self-Management Education Program. The program will start with an assessment by a Certified Exercise Specialist to develop a safe personal exercise plan meeting your diabetes and health concerns. A prescription from your physician is necessary.

Cost: $27.00
Exercise class options include: Walk Down Your Blood Sugar and Take Control with Exercise. A class fee applies.
Exercise Sessions are available two to three days per week. The exercise group meets in
the Cardiac Rehabilitation Area for supervised sessions. Cost: $32.00 monthly

Medical Nutrition Therapy
Individual appointment with a Registered Dietitian to meet your specific nutritional needs. Areas addressed can include heart health, kidney disease, weight management, diabetes, or other medical concerns. Fee may be covered by insurance. Appointments are available at Monroe or Dundee.

Continuing Diabetes Education (follow-up training)
This program is designed for individuals that have already attended the comprehensive diabetes education program. The classes provide updates on important diabetes topics and include: a diabetes refresher, testing smart, heart disease and coping with diabetes. Nutrition updates include: a nutrition refresher, advanced carbohydrate counting and nutrition for heart disease. Most insurance plans cover the cost of education.
