How can I obtain a copy of my birth certificate?
In order to obtain a copy of your birth certificate, you must contact the Monroe County Clerk's Office at 734-240-7020.
How do I obtain a copy of my medical record?
In order to obtain a copy of your medical record, a request authorization form must be properly filled out and provided to the Health Information Management Department. Please specify what part of the medical record you would like to receive, for example, Discharge summary, History and Physical, Lab results, etc.
When picking up your medical record you must provide valid photo identification or you will not be given a copy of your medical record.
How long will it take Mercy Memorial Hospital to retrieve my medical record once I request it?
The medical record may be retrieved same day when the request is received via fax or in person. If the request is received via mail the expected retrieval time is 3 to 5 business days from the date the request is received. If a requested medical record is 7 years old or older, the retrieval time is 7 to 10 days, regardless of how the request is received.
How much does it cost to receive a copy of my medical record?
Each patient can request one copy of their medical record free of charge. Each additional request will be charged as follows, the initial retrieval fee is $21.20, plus $1.06 per page for pages 1-20, $0.53 per page for pages 21-50, $0.22 per page for pages 51 +.
Can I request a copy of my spouse's medical record?
You are ONLY able to obtain a copy of your spouse's medical record if you are able to prove that you are the legal guardian over your spouse. In order to prove guardianship, you must submit a valid proof of guardianship to the Health Information Management Department.
I am my spouse's Power of Attorney; can I have access to my spouse's medical record?
In order to obtain a copy of your spouse's medical record, you must submit a valid proof of Power of Attorney. The Power of Attorney papers must specify that you have the power to manage health care decisions, authorize medical treatment, and conduct affairs related to health care. Once the Power of Attorney papers are provided, you must submit a signed authorization form. When picking up the copy of the medical record you must provide your valid photo identification or you will not be given a copy of your medical record.
How do I obtain a copy of my x-ray reports or my lab results?
In order to obtain a copy of these reports, you should follow the guidelines for a medical record request and just specify which report you would like a copy of such as CAT scan, x-ray of leg, or lab results.
How do I obtain a copy of my x-ray films or lab diagrams?
In order to obtain a copy of your x-ray films you must contact the Imaging Services Department (Radiology) at 734-240-5600.
Do I have to come to the hospital to pick up a copy of my medical record or can the Health Information Management Department just fax or mail me a copy?
You must come to Health Information Management Department to pick up the copy of your medical record. The reason you are required to come to the Health Information Management Department to pick up your medical record is so we can check your photo identification to ensure we are providing a copy of your medical record to the person specified on the authorization form.
