Frequently Asked Questions

How do I obtain a copy of my medical record?
In order to obtain a copy of your medical record, a request authorization form must be properly completed and returned to Mercy Memorial Hospital System. Please specify what part of the medical record you would like to receive, for example, discharge summary, history and physical, lab results, etc. When picking up copies of your medical record, you must provide valid photo identification. If you do not have valid photo identification, you will not be given a copy of you medical record.

How long will it take to receive my medical record once I request it?
The medical record will be retrieved and available for pickup in two business days. If the requested medical record is prior to the year 2002, the retrieval time is seven to 10 days. If for some reason we find that it will take longer than this to retrieve your record, our Medical Records department will contact you.

When can I pick up my medical record?
You can pick up your medical record two business days from the date requested. In order to pick up your medical record, please proceed to the cashier’s window in the registration area. Your record will be available for pick up Monday through Friday from 7:30 a.m. to 5 p.m.

Can my medical records be mailed to me?
Yes. In order for your medical records to be mailed, you must request the records in person. Upon request, you must provide valid photo identification and specify on the authorization form that you would like your records mailed to you. The records will be mailed within two business days.

How much does it cost to receive a copy of my medical record?
For personal use, each patient can request one copy of their medical record free of charge. Each additional request will be charged as follows, the initial retrieval fee is $21.20, plus $1.06 per page for pages 1-20, $0.53 per page for pages 21-50, $0.22 per page for pages 51+.

Can I request a copy of my spouse’s medical record?
In order to obtain a copy of your spouse’s medical record, you must be able to prove that you are the legal guardian over your spouse. In order to prove guardianship, you must submit a valid proof of guardianship to the Health Information Management Department.

I am my spouse’s Power of Attorney; can I have access to my spouse’s medical record?
In order to obtain a copy of your spouse’s medical record, you must submit a valid proof of Power of Attorney. The Power of Attorney papers must specify that you have the power to manage health care decisions, authorize medical treatment and conduct affairs related to health care. Once the Power of Attorney papers are provided, you must submit a signed authorization form. When picking up the copy of the medical record, you must provide your valid photo identification or you will not be given a copy of the medical record.

How do I obtain a copy of my x-ray films or lab diagrams?
In order to obtain a copy of your x-ray films you must contact the Imaging Services Department (Radiology) at (734) 240-5600. In order to obtain lab diagrams, you must have your physician fax a request to the lab department at (734) 240-5801. The lab department will then process your request. You will be required to follow the guidelines for requesting a medical record.
