The Health Information Management Department is dedicated to providing the best possible service for the patients at Mercy Memorial Hospital System. The HIM professionals in our department utilize their specialties to work diligently as one centralized unit to manage patient's information accurately and completely. We are here to ensure the quality and privacy of each patient's medical health record and to work with MMHS's medical staff for continuous improvement of how data is processed and managed.

Our Health Information Management Department is equipped with secured systems, coding/dictation software, and a premier tumor registry system. We ensure that we operate on the leading edge of industry standards by updating our systems and processes. We also practice QCI (quality control indexing) for each patient's record. Our HIM department's goal is to:

• Maintain the standards and guidelines set by HIPAA and the Joint Commission
• Improve the quality of care
• Strengthen the relationship with the community
• To achieve excellence in information management and operations

To request a copy of your medical record, please complete the Authorization for Release of Patient Medical Information form. After you have printed and completed the form, please bring it to MMHS’ Information Desk located in the lobby or mail it:
