How to Report a Patient Care or Safety Concern

If you feel strongly that there is a systemic safety or quality issue that may have not been addressed, you may:
- Call The Michigan Department of Community Health at (800) 882-6006
- Call The Joint Commission at (800) 994-6610
- Contact The Joint Commission by email
- Go to The Joint Commission website (www.jointcommission.org), print a Quality Incident
Report form, write out your concern and fax or mail to The Joint Commission. The fax
